For a shelter location in your state, contact your state's division of emergency management. FEMA has a list of contact information here: http://www.fema.gov/about/contact/statedr.shtm. You can also search for a shelter on the Red Cross' website: http://app.redcross.org/nss-app/

American Red Cross recommends, at a minimum you should have basic supplies in an easy to carry emergency preparedness kit that you should have at home or to take with you.
For details on what to bring with you to a shelter, visit: http://rdcrss.org/12qt24

When you evacuate, where should you and your family go? Should you bring your evacuation kit or could you just leave with food, water, clothes, etc.?
